Step 3

Submit & Pay Co-Pay

Fill out the application with the provider. Select "Medicaid" as your qualifying program. Upload your documents.

Be prepared to pay the federally mandated co-pay ($10-$50) for the device. This is required by law.

Official Verification

Your application goes through the National Verifier. This system checks federal and state databases. Because Medicaid is a state program, sometimes the automatic check fails.

Don't Panic! If it fails, you simply need to upload that Benefit Verification Letter we mentioned earlier.
